WAIVER OF LIABILITY:

PLEASE READ CAREFULLY- Waiver of Liability: I/We acknowledge that though all precautions are taken to ensure participant safety, this is a cooking camp, where campers will use appliances and utensils such as, but not limited to: oven, stove, knives, microwave, toaster, beaters, electric frying pans, cleaning chemicals, etc. I/We are aware that if my child does not follow the safety precautions taught in the program, or an accident happens, they may be injured. I/We also acknowledge that Creative Culinary Minds strongly recommends keeping current my child's tetanus immunization. I/We acknowledge that it is up to the discretion of the Head Chef (Diane) to remove my child from the program at any time if my child fails to respect the rules of the program and the safety of others. If this occurs, I/we acknowledge that I/we will be expected to remove my child from the program as soon as possible. As Creative Culinary Minds is an inclusive space, I/We acknowledge the importance of being open with the Head Chef, with the needs of my child, along with any information that would make my child more comfortable in the program. I/We understand that it is my responsibility to disclose any behavior or medical concerns that may impact my child's participation in the program at Creative Culinary Minds in advanced of the program. Failure to disclose behavioral or medical concerns may result in my child not fully enjoying the program. In consideration of my child participating in the Summer Camp with Creative Culinary Minds, I/we, the undersigned, agree to assume all risk of loss or injury for my child while my child is participating in the program, and hereby waive any rights of action I/we may have had or may in the future lodge against Creative Culinary Minds, its successors, assigns, directors, staff, agents or volunteers or any such loss or injury whether caused by the negligence or default of Creative Culinary Minds, its successors, assigns, directors, staff, agents or volunteers. I/We understand that Summer Camp runs on Monday-Friday from 9:00am-4:00pm. I/We understand that the location for Summer Camp is at Centennial Collegiate Vocational Institute, located at 289 College Ave West, Guelph, ON and it is my responsibility to sign my child in and out upon pick-up and drop-off. I/We understand that Creative Culinary Minds or its assigns may take photographs of my child participating in activities. These photographs may be used for future promotional and marketing purposes, like website, social media, pamphlets or posters. Refunds or exchanges may be requested prior to June 19, 2019. Any refund or exchange requested prior to June 19, 2019 at 9:00am will be subject to a 20% administration fee. After said time, refunds will be issued at the discretion of Creative Culinary Minds. Please ensure that your order is correct before submitting it. Please send your e-transfer of 50% of your child’s registration fee immediately to creativeculinaryminds@gmail.com .
